PLEASE READ THE NOTES BELOW AND THE ELIGIBILITY CRITERIA BEFORE COMPLETING THE APPLICATION FORM

Please note that the application form for the 2009 Fellowships has been built specifically for the use of PCs only. Anyone wishing to apply on a MAC will have to source a PC to apply, or contact the Trust to fill out a hand written application.

	Please type directly onto the application form rather than 'cutting and pasting' from other applications. Also DO NOT FILL IN THE FORM USING CAPITAL LETTERS, please use Upper and Lower case only. Also DO NOT use any formatting such as bullet points, the Trust is only interested in the content submitted within the form NOT the layout.
	This form was designed using the Adobe Acrobat. Other browsers may show the form slightly differently. The form has not been devised specifically for Apple Macintosh's, if you would like to apply online, please use a PC.
	As the selection process involves many applications and much photocopying, each of the text boxes contains a limited number of characters. The application form will not allow the creation of additional space.
	DO NOT print out your completed form for forwarding, by post or fax, to the Trust. It will NOT be accepted in that format; in other words, the 'on-screen' application form will only be accepted 'on-line' - by using the 'Submit' button at the bottom of the form.
	The 'Submit' button should only be used once to avoid duplicating your application. A message will appear on the screen informing you that your submission has been sent and a flattened PDF version of the form will be emailed to the email address you used to register.
	Under "BRITISH NATIONALITY", you are confirming that you are a British Citizen under the terms of the British Nationality Act 1981. You must be resident in the UK.
	Applications are considered for individual projects only; joint or group applications will not be accepted. Applications for Fellowships of under 4 weeks will not be considered.
	The information on this form will enable the Trust to draw up a shortlist for interview. Those selected will be invited to complete a further form expanding on their plans outlined on this form.
	If you are short listed, you will be required to provide 2 references in support of your application.
	You will receive confirmation of your application from the Trust within 7 to 10 working days stating your Trust reference number. If you are in doubt, please ring the Trust Office, 020 7584 9315.
