An exploration of comprehensive cancer survivorship care models

There is no formalised method of follow up care for patients who have completed treatment for haematological cancers in the UK. Australia and Canada have well established models of follow up care for cancer survivors. With increasing number of survivors and possible long term side effects that could impact on the quality of life it is important to formalise follow up care and provide a seamless coordination between health care providers and travel to these centres did help address this need.
